Michigan Care Improvement Registry (MCIR) Updates

The Michigan Care Improvement Registry (MCIR) continues to undergo significant changes aimed at modernizing Michigan’s immunization infrastructure, improving data accuracy, and supporting more efficient vaccine reporting and record management. On August 10, 2026, the Michigan Department of Health and Human Services (MDHHS) launched the new MCIR platform, powered by STC|ONE, an immunization information system developed by STChealth. This transition represents a major modernization effort intended to enhance system functionality, interoperability, and the exchange of immunization information among health care providers and public health partners..

Since implementation, however, there have been several challenges, including missing or inaccurate immunization records, delays in messaging, and difficulties with Vaccines for Children (VFC) inventory reconciliation. These issues have created administrative burdens and workflow disruptions for practices that rely on MCIR for accurate immunization documentation and reporting. MDHHS continues to work with its technology partner, STChealth, to address identified concerns, implement system improvements, and provide additional training and technical assistance.

The transition has also affected schools and childcare providers in assessing and reporting student immunization status. Due to concerns with inaccurate or incomplete immunization records and system-generated assessments, MDHHS issued guidance in October advising schools and childcare providers to temporarily refrain from generating MCIR immunization reports and letters. These issues have resulted in some students incorrectly appearing noncompliant with school immunization requirements despite having received the necessary vaccinations. Schools have been encouraged to focus on verifying system access and reviewing student rosters while the issues are addressed. Students should not be excluded from school solely because of inaccurate or incomplete MCIR records.

The MCIR reporting page continues to list November 1, 2026, as the school immunization reporting deadline; however, current guidance instructs schools and childcare providers not to run MCIR reports or letters until additional guidance is provided. Practices should be prepared to assist schools and families with verification of immunization records while MDHHS works to restore accurate reporting functionality. MDHHS is expected to provide additional guidance as system issues are resolved.

Physicians and their practices are encouraged to continue submitting immunization information, monitor the accuracy of patient records, and stay informed about updates that may affect reporting workflows and compliance requirements. Additional information, system updates, and resources are available through the MCIR Modernization website.
