March of Dimes Provider Survey
March of Dimes in conjunction with Michigan Public Health Institute has developed a survey to understand access to alpha-hydroxyprogesterone (17P) and barriers that providers and consumers face when utilizing 17P as prevention for recurrent preterm birth. The purpose of this survey is to collect information from providers about their experience with prescribing and administering 17P. The data collected will be used by March of Dimes to guide efforts to address barriers to 17P and improve services for patients.
This survey will take approximately 10-15 minutes to complete. Your participation in this survey is completely voluntary. Your answers are confidential. The survey data will be managed and analyzed by Michigan Public Health Institute.
