RESOLVED: That MSMS ask the Michigan Health Insurance Commissioner to require all health insurance companies and/or pharmaceutical benefit managers to include the appropriate prior authorization forms with the faxed notification and/or to include a web address where prior authorization forms may be easily accessed by the physician; and be it furtherRESOLVED: That MSMS ask the Michigan Health Insurance Commissioner to work with health insurance companies and pharmacy benefit managers to develop simple prior authorization forms which will be used uniformly by all of the health insurance companies and pharmacy benefit managers in this state.
