MI Health Link Provider Summit: Strategies for Coordinating Care

The Michigan Department of Health and Human Services (MDHHS) will be hosting a Provider Summit on MI Health Link at the Kellogg Hotel and Conference Center on Wednesday, November 9 from 8 am to 4:30 pm.

Sessions will cover a range of topics associated with integrated health care and person centered planning. Join us to discuss:

  • Person centered planning
  • Effective care coordination
  • Home and Community Based Services (HCBS)
  • Best Practices for Integrated Care Teams
  • Enrollee success stories and best practices

Hosted by MDHHS in partnership with the Michigan State Medical Society, Health Care Association of Michigan, LeadingAge Michigan, Michigan Association of Community Mental Health Boards, Michigan Association of Health Plans, Michigan Assisted Living Association, Michigan Center for Assisted Living, Michigan County Medical Care Facilities Council, Michigan Health & Hospital Association, and Michigan Primary Care Association.
