SIM PCMH Intent to Participate

Last month, the Michigan Department of Health and Human Services' (MDHHS) State Innovation Model (SIM) team announced that the Patient Centered Medical Home Initiative would be launching an Intent to Participate process for potentially eligible providers/practices to express their interest in engaging in the Initiative. The Intent to Participate (ITP) process is now open and available online. The ITP process, which takes approximately 20-30 minutes to complete, will be open for providers until Thursday, June 30th at 5:00 pm.

The ITP collects a minimal amount of information in a survey format and then provides a downloadable Excel spreadsheet for responding providers/practices to complete and upload within the online survey system. The ITP is designed to assist the SIM Team in identifying the number of interested providers, the populations served by those providers/practices and the practice capabilities in place to build from as part of the PCMH Initiative. This information will be used in stakeholder engagement efforts going forward as well as PCMH Initiative operationalization. A formal application, and participation agreement for providers/practices selected to be part of the Initiative, will follow the ITP process later this year.

While the Intent to Participate process is not binding (i.e. a provider/practice can choose not to apply after submitting their intent), it is required for providers/practices with interest in the PCMH Initiative. Practices must submit the ITP to be invited to complete the full application and participation agreement later this year.

The Intent to Participate process launching today is specifically for providers/practices in two eligible groups described below for the PCMH Initiative launching January 1, 2017. A general description of the PCMH Initiative is attached to this email; more specific Initiative details and participation parameters will be released prior to the formal application process. The SIM team will open additional ITP processes in advance of PCMH Initiative expansions to other portions of the state planned for subsequent participation years.

2017 Eligible Provider/Practices

  • Providers/practices physically located in one of the five SIM regional test locations, which include: Genesee County; Jackson County; Muskegon County; Northern Michigan, which includes the following counties: Antrim, Benzie, Charlevoix, Emmet, Grand Traverse, Kalkaska, Leelanau, Manistee, Missaukee and Wexford; Washtenaw and Livingston Counties   
  • Currently participating Michigan Primary Care Transformation Project (MiPCT) providers/practices located anywhere in Michigan (The ITP process has been streamlined for practices currently participating in MiPCT and may take less time to complete due to previous collection of similar data from MiPCT practices)

If a provider/practice is affiliated with a Physician Organization or other integrator organization, the organization may complete this form on behalf of its affiliated/member practices. Integrator organizations must obtains consent from the practice(s) and designate a primary contact from the practice(s) to receive subsequent communication and correspondence in addition to the integrator organization itself.
