The Lake Superior Quality Innovation Network (LSQIN) is inviting physician organizations, individual providers, home health agencies, hospitals, skilled nursing facilities, hospices, palliative care organizations and nursing homes to participate in their quality improvement work. 

The LSQIN is comprised of three quality improvement organizations - MPRO of Michigan, MetaStar of Wisconsin and Stratis Health of Minnesota. These organizations have partnered to establish a regional network to promote and lead Centers for Medicare & Medicaid Services (CMS) quality improvement initiatives. The LSQIN work is grounded in foundational principles that align with the goals of the CMS Quality Strategy: eliminating disparities; strengthening infrastructure and data systems; enabling local innovation; and fostering learning organizations. 

The work provides an opportunity to participate in multiple quality improvement initiatives that help optimize the health care that's delivered and its positive impact on patients. LSQIN is offering initiatives to improve cardiac health, reduce disparities in diabetes care, increase prevention coordination through Meaningful Use of health information technology, reduce hospital readmissions, improve quality through value-based payment and quality reporting, reduce healthcare-associated infections in hospitals and reduce healthcare-acquired conditions in nursing homes. Together, these initiatives represent an integrated combination of sustainable activities designed to help drive system-wide quality improvement. There is no cost to participate.
