The UM Health Plan, formerly the Physicians Health Plan (PHP) announced that it will close at the end of 2025.  PHP serves about 64,000 individuals primarily in the Mid-Michigan area.  The UM Health Plan has instructed policy owners to transition their health plans at their 2025 renewal.
