Auto No-Fault Survey

Michigan Public Health Institute (MPHI), on behalf of Brain Injury Association of Michigan (BIAMI) is conducting a survey to understand how the new auto insurance law is impacting crash survivors and their families, as well as service providers. The aggregate information will be shared with lawmakers and other stakeholders to better understand the effects of the 2019 changes to Michigan Auto Insurance Law including those that took effect July 1, 2021 on the healthcare systems and patients in our state. We are encouraging you to participate in this very important survey. To track the changes over time, a series of three online surveys will be conducted September 2021 to April 2022. We invite you to participate today in the first survey in the series.

For survivors and/or family members, the survey will ask questions about services you or a family member received before and after July 1, 2021. Additionally, the survey will invite you to provide some demographic information, including personally identifiable information and some health information. Completing the survey on average should take approximately 30 minutes.

For service providers, the survey will ask questions about the services your organization provides and how the changes have impacted your operations. Completing the survey on average should take approximately 15 minutes.

Your survey answers will be collected by MPHI where data will be stored in a password protected electronic format and can be accessed only by authorization. MPHI takes extreme care to protect sensitive data.

There will be no incentives provided to you as a respondent other than the satisfaction of knowing that you contributed to this landmark initiative.

To ensure that all those affected by the changes in the law are represented in the survey responses, we urge you to share the survey link with contacts that have the potential to contribute.

Please submit your completed survey no later than Wednesday, October 13,  however, it would be greatly appreciated if you complete as soon as possible.

Instructions on completing the survey:

  1. To open the survey, click the link Auto Crash Survivors and Service Providers Survey,
    or copy into your web browser
  2. If this is your first attempt, just follow through the questions to the end (the last step is to click “Submit”). If you need to stop and continue later, click  “Save & Return Later” at the bottom of the page you are working on and you will be given a Return Code. Please write down the Return Code.
  3. If you want to continue from where you left and you have a Return Code, open the survey the same way, then click “ Returning?” on the upper right corner of the first page (the consent page), enter the return code as instructed. You can modify your answers by clicking “<>”
  4. If you have forgot your Return code, please Email to request your Return Code be emailed to you (you would need to use the same email you provided in the survey for us to identify your Return Code)
